We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,400 providers in Indianapolis, IN. This opportunity is in Kokomo, IN, on the campus of Community Howard Regional Hospital at 3611 South Reed Road.

Community Health Network, in partnership with Jane Pauley Community Health Centers, has an excellent opportunity for a pediatric medicine or family medicine trained and certified advanced practice provider to work with our outpatient pediatrics practice in Kokomo, IN.

· A multi-disciplinary team equipped to provide vital resources to patients, including community resource navigators

Licensure and Certification

  • Educational background consistent with requirements for licensure and certification within the state of Indiana. Must successfully complete a nationally recognized Advanced Practice Nurse certification or PANCE or have current national certification; must maintain certification on an ongoing basis.

  • Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; or the Indiana State Physician Assistant Committee; must maintain eligibility for prescriptive authority on an ongoing basis.
